Diffusion-weighted imaging with adc-maps in assessement of liver transarterial chemoemboli-zation response in patients with metastatic neuroendocrine tumors.

Рurpose. To determine the value of diffusion-weighted MR-images (DWI) with apparent diffusion coef-ficient (ADC) maps for liver transarterial chemoembolization (TACE) efficiency assessment in patients with metastatic neuroendocrine tumors (mNETs). Material and methods. 31 liver mNET patients that undergone 61 TACE procedures were enrolled in the study. 108 MRI studies performed before and after treatment, including DWI with ADC-maps and T1-WI, before and after contrast enhancement, were analyzed. TACE effect was estimated according to RECIST 1.1. Before and after TACE, we measured: maximum necrotic/fibrotic zone diameter and solid component thickness at the lesion, ADC and its standard deviation (SD) in the tumor solid component. Results. TACE effect was evaluated according to RECIST 1.1 as progressive disease in 4, stabilization in 54 and partial response in 3 patients. No statistically significant differ-ence in time to progression between the stabilization and partial response groups was found. The diameter of the fibrotic/necrotic areas after TACE demonstrated no changes, thickness of the solid component on its periphery decreased. ADC values and its SD in-creased after TACE, and it is positive prognostic factors with a connection to time to progres-sion. Structural changes in the tumor tissue may be characterized by an increasing of ADC, which allows using DWI with ADC-maps for the early detection of tumor changes. Conclusion. The usage of DWI with ADC-maps leads to an increase in the nonpro-gression prognostic value of MRI in TACE efficiency assessment in patients with mNETs.
