Purpose. Demonstration and description of the extremely rarely cardiac abnormality - congenital left atrial aneurysm. Materials and methods. The patient, 64 year old, was admitted to the clinic with dyspnea and palpitation. Pulmonary embolism was suspected. MSCT-pulmonography, cardiac sonog-raphy and MRI were performed. Results. Diagnosis of pulmonary embolism confirmed. A giant aneurysm of the left atrium was identified. Conclusions. In most cases, it detected while finding reasons for supraventricular arrhythmias and thromboembolic sources. In some patients, this is accidental finding during routine examination. The most serious complications include cerebral thromboembolism and atrial fibrillation. The standard treatment strategy is surgical resection of the aneurysm. This case is a two years’ follow up without surgical treatment. There were no complications.
