Opportunities of CT-perfusion in the evaluation of renal blood flow in patients with urolithiasis

Purpose. The purpose of this study was to investigate the hemodynamic changes in the cortex and the medulla of the kidneys in patients with various forms of urolithiasis (ICD), using CT-perfusion performed on 640 sectional computed tomographs.

Materials and methods. From 2017 to 2019 in the Russian-Japanese center of visualization of the Sechenov University 42 patients with various forms of urolithiasis were examined. CT-perfusion was performed in all patients with urolithiasis at the preoperative stage. The study was performed on a Aquilion One 640 scanner (CANON, Japan) in a volume mode, with a slice thickness of 0.5 mm.

Results. According to CT-perfusion data in patients with ureteral stones with dilatation of CLS there were reduced perfusion parameters (AF and BV) in the cortical and medulla, while the perfusion values in the contralateral kidney were statistically larger than the average perfusion values. In patients with kidney stones (coral stones and stones larger than 1 cm), a decrease in perfusion indices was observed only on the affected side. Changes in the perfusion values of the contralateral kidney were noted.

Conclusion. CT-perfusion is an accurate and non-invasive method that allows you to quantify the functional status of the kidney, the degree and nature of the obstruction, which can be clinically significant when choosing a method of surgical treatment, prediction of complications and further dynamics of urolithiasis.
