Purpose. To assess the atrophic process of the brain in progressive supranuclear palsy according to magnetic resonance morphometry and identify the most specific areas of the brain atrophy to improve the accuracy of diagnosis.

Materials and methods. The results of magnetic resonance morphometry of patients using postprocessing of native MR-images on Freesurfer software were analyzed. 26 patients with progressive supranuclear palsy were examined. As a control group, patients without relevant neurological pathology of the appropriate age were examined.

Results. Distribution of the atrophic process both in the structures of the brain stem and in the brain cortex in the progressive supranuclear palsy.

Discussion. The results of the study correlate with the data obtained earlier, while adding new data on the broader involvement of the cerebral cortex in the neurodegenerative process. Previously published results indicate atrophy of the stem structures, subcortical ganglia, frontal and parietal lobes of the brain.

Summary. Methods of postprocessing of MR morphometry data allow instrumental confirmation of the clinical diagnosis, as well as to clarify the pathogenesis of neurological syndromes observed in progressive supranuclear palsy.
