Acute myeloid leukemia is a neoplastic disease characterized by uncontrolled clonal proliferation of abnormal myeloid stem and progenitor cells in the hematopoietic tissue. The clinical diagnosis of acute myeloid leukemia is challenging, due to its varying presentation and nonspecific symptoms.

Purpose. To show difficulties in acute myeloid leukemia with bone manifestation, using case report.

Materials and methods. A 58-year-old patient presented with long-term subfebrile condition, low back pain and weight loss. CT of the chest, abdomen and pelvis revealed multiple bone lesions. Trephine biopsy was performed. According to the findings of trephine biopsy, myeloid leukemia was suspected.

Conclusion. The clinical case demonstrated an atypical presentation of acute myeloid leukemia in the adult patient, clearly illustrating the importance of correct data interpretation and the choice of a diagnostic algorithm.
