Purpose. To highlight the issues of differential diagnosis of diffuse alveolar hemorrhage syndrome (AHS) and cardiogenic pulmonary edema as far as difficulties in treatment of patients with critical aortic stenosis (AS) when escalation of antithrombotic therapy is needed.

Materials and methods. The authors present  the  clinical  case  of 80 years old female pa-

tient with critical AS and mechanical mitral valve prosthesis scheduled for transcatheter aortic valve implantation (TAVI). The patient had been given triple   antithrombotic therapy due to urgent percutaneous coronary intervention with drug-eluted stent implanted at the time of acute myocardial infarction.

Results. The clinical and rentgenological features of AHS in the patient of cardiovascular department are presented. Using of multiple antithrombotic therapy may cause AHS in patients with advanced pulmonary hypertension.

Conclusion. The key role of radiologic imaging modalities, including chest X-ray and computed tomography, in diagnostics of AHS is focused on in the paper.
