CLINICAL ASPECTS OF CARDIOVASCULAR DISEASES

urpose. To study the characteristics of pericardial fat and their relationship with clinical and laboratory parameters in patients with coronary heart disease (CHD) and patients without cardiovascular diseases (CVD).
Patients and methods. Data analysis of 186 patients was performed: 113 – with CHD and 73 – without CVD. The computed tomography was used with scans in axial projection and mediastinal mode to measure the parameters of pericardial fat (PF). Statistical analysis was performed using the SPSS v.23 program. Student's criterion, Pearson (r) and Spearman (rs) correlation coefficients were applied.
Results. The volume and density of the PF in patients with CHD were significantly higher than in patients without this pathology. Thus, the volume of the PF was 92.66±41.66 cm3 versus 55.30±32.16 cm3, p<0.001, fat density was 64.46±24.71 HU versus -84.90±34.59 HU, p<0.001. In patients with atrial fibrillation (AF), the values of the volume of the PF were greater: 103.29±44.49 cm3 versus 85.63±38.41 cm3, p=0.027. Among patients taking lopid-lowering drugs, the indices of PF density were lower: -73.60±21.28 HU and -62.15±21.68 HU, p = 0.012. Positive correlations of PF volume with BMI (body mass index) were found (r = 0.698 (p<0.001) and r = 0.384 (p = 0.009)) and glucose (rs = 0.242 (p = 0.049) and r = 0.303 (p = 0.043)) in men and women, respectively. Negative correlations of density with BMI (r = -0.247, p = 0.045), total cholesterol (r = -0.258, p = 0.038), triglycerides (r = -0.271, p = 0.036), HDL (r = -0.302, p = 0.032) were found only in men.
Discussion. The metabolic activity of the PF is associated with changes in the expression of proatherogenic and profibrotic genes. This provides a background for the formation of CVD.
Conclusion. The volume and density of the pericardial fat tissue are different in patients with and without coronary artery disease. A possible effect on the density of pericardial fat was revealed due to lipid-lowering therapy in CHD. Quantitative changes of the pericardial fat in AF were determined. Correlations of PF characteristics with clinical and laboratory parameters were found.
