DUAL-ENERGY CARDIAC COMPUTED TOMOGRAHY AS AN ALTERNATIVE IMAGING METHOD OF THE LEFT VENTRICLE STRUCTURAL ANOMALY

he study of myocardial fibrotic remodeling markers determines the growing interest of non-invasive cardiac imaging.
Purpose. To provide the reliability data of CT measurement of ECV by employing MRI T1 mapping as a gold standard.
Materials and methods. The study included 3 patients groups (n=17) mid.ages 56: with HCM (n=8, 47%), с IHD (n=5, 29%) and myocarditis (n=4, 24%). Extracellular volume fraction (ECV) of left ventricular myocardium was measured both by DECT and by MRI in 204 segments (102 segments in DECT+ 102 segments in MRI x 17).
Results. DECT/ECV values data were overestimated by an average of 13,6% compared to ECV/MRI: average ECV/DECT value over 102 segments was 30.18% ± 2.8% and ECV/MRI over 102 segments 26.6% ± 2 .02%, with no statistic difference (p=0,001). Bland Altman Bland-Altman analysis of the agreement between DECT and MRI showed only a small bias 0,2±0,05% with 95% limits of agreement of 3.49% to 2.73%. Interobserver agreement was excellent (ICC = 0.89). Were found the differences in groups of patients with HCM (r=0.78, p<0.001), IHD (r=0.89, p<0.001) and myocarditis (r=0.9, p<0.001). Correlation were close with ECV/DECT and LVEDV/DECT (r=0.37, p=0.13), and with LVEF/DECT (r= -0.21, p=0.4).
Conclusions. DECT could provide accurate myocardial ECV measurements and might be a useful approach for the clinical assessment of cardiac diseases.
