RADIOLOGICAL DIAGNOSIS OF CRITICAL CONGENITAL HEART DEFECT – TOTAL

ANOMALOUS PULMONARY VENOUS RETURN IN A 1-DAY-OLD CHILD

o demonstrate the capabilities of multslice computed tomography (MSCT) in detecting and detailing total anomalous pulmonary venous return (TAPVR) in comparison with transthoracic echocardiography.

Materials and methods. A clinical observation of a newborn patient with suspected congenital heart disease (CHD) in the first hours of life is presented. The patient underwent transthoracic echocardiography and MSCT of the chest, abdominal cavity and retroperitoneal space in native phase and with bolus contrast enhancement in order to: 1. clarify the diagnosis; 2. determining patient management tactics.

Results. When conducting a transthoracic echocardiography in a child with congenital heart disease, a single collector of all pulmonary veins (PVs) draining into the portal vein was visualized, and the diagnosis was made – congenital heart disease. TAPVR with the entry of the common PV collector into the portal vein, atrial septal defect (ASD). For the purpose of preoperative planning, to detail the defect as well as to exclude possible other anomalies and complications associated with congenital heart disease, CT-AG of the chest and abdominal cavity was performed.

Discussion. Transthoracic echocardiography has high diagnostic accuracy in the diagnosis of TAPVR, however, this method is limited in visualizing extracardiac complications and therefore is not the final and preferred diagnostic method.

Conclusion. A multimodal approach should be applied to the diagnosis of CHD, which is aimed at assessing both the anatomical and functional parameters of the heart and extracardial structures.
