METASTASIS OF PROSTATE ADENOCARCINOMA INTO THE CONDYLAR PROCESS HEAD OF THE LOWER JAW

Purpose. Demonstration of the capabilities of multislice computed tomography (MSCT) in the diagnosis of a rare case of metastasis of prostate adenocarcinoma to the condylar process head of the mandible two years after the surgical treatment and in the absence of metastases to other organs.

Materials and methods. We present a clinical observation of patient M., 64 years old, who was admitted to a maxillofacial surgeon for consultation with complaints of moderate pain in the area of the left temporomandibular joint, limited mobility of the lower jaw; these complaints were present for several months. In order to identify changes in the temporomandibular joints, the patient underwent multispiral computed tomography (MSCT), as well as osteoscintigraphy.

Results. According to the medical history, two years ago the patient underwent a radical prostatectomy for prostate adenocarcinoma (cT2cN0M0), no data for recurrence was obtained during the annual examination. Based on the results of MSCT of the temporomandibular joints, the patient was diagnosed with a lesion in the head of the condylar process of the mandible on the left. A puncture biopsy was performed: according to morphological and immunohistochemical characteristics, this lesion corresponded to a metastasis of acinar adenocarcinoma of the prostate gland. MSCT of the chest and abdominal organs was performed, osteoscintigraphy – no data on the presence of a metastatic lesion of another localization was obtained. The patient was referred to an oncologist to select further treatment tactics.

Discussion. This clinical observation describes an extremely rare localization of single distant prostate adenocarcinoma metastasis which was the first manifestation of the metastatic process. Prostate adenocarcinoma tends to early metastasize through the hematogenous route to the liver, lungs and skeletal bones. Metastatic lesions of the head of the condylar process of the mandible are extremely rare due to the small amount of bone marrow in the condyle and the blood supply by the circular perforating branches of the maxillary artery and the superficial temporal artery.

Conclusion. The presented clinical observation demonstrates the importance and necessity of conducting multislice computed tomography in patients with a history of surgical treatment for prostate adenocarcinoma, in order to identify possible distant metastases, including such a rare location as the lower jaw.
