Purpose. To present a rare clinical case of cholelithiasis that caused an acute intestinal obstruction.

Materials and Methods. The article presents a clinical case of a 69-year-old woman with gallstone small bowel obstruction. We present the capabilities of computed tomography in the diagnosis of Rigler's triad, which is described as a combination of radiological signs characteristic of intestinal obstruction caused by gallstones.

Results. Data from an objective patient examination made it possible to suspect the manifestation of intestinal obstruction, and radiological examination methods made it possible to accurately determine the cause of the disease and plan the optimal scope of surgical intervention.

Discussion. The use of radiological methods is important for diagnosing this disease. Computed tomography (CT) allows determining the exact localization of obstruction, bile-intestinal fistula, and the size of the ectopic stone. CT has a high sensitivity for signs of Rigler's triad. Accurate preoperative diagnosis using CT plays an important role in the choice of treatment method.

Conclusion. Gallstone ileus is a rare condition with a non-specific clinical presentation. This case demonstrates that CT scanning is a powerful tool for the diagnosis of gallstone ileus. Precise preoperative diagnosis with CT plays a significant role in decision management regarding treatment.
