Purpose. To evaluate the role of CT in planning of transcatheter tricuspid valve replacement (TTVR) using the Lux-Valve Plus prosthesis and to present the first experience with this technology in Russia.
Materials and Methods. Based on the first domestic experience with the Lux-Valve Plus, a comprehensive examination was performed on an 84-year-old patient with severe secondary mitral regurgitation (grade 4–5), high surgical risk (EuroSCORE II 8.6%), and comorbidities, including echocardiography and cardiac CT (Canon Aquilion ONE Genesis). A detailed analysis of the tricuspid annulus, anchoring zones, and vascular access was performed.
Results and discussion. Preoperative CT scans were used to determine the main anatomical parameters of the tricuspid valve and the right ventricular chambers: the dimensions of the annulus, its circumference, area, and thickness of the interventricular septum; the angle between the plane of the tricuspid annulus and the septum; and the length of the right ventricle. The diameter of the internal jugular vein and the distance from the fibrous annulus to the right coronary artery were determined. Following an interdisciplinary consultation, the Lux-Valve Plus prosthesis, size 60 mm, was selected. Successful transcatheter implantation of the tricuspid valve prosthesis via a transjugular approach was performed. According to follow-up echocardiography, the severity of mitral regurgitation decreased from grade 4–5 to grade 2, and positive clinical dynamics were noted.
Conclusion. CT is an important step in preoperative planning for transcatheter tricuspid valve replacement, allowing for an accurate assessment of the anatomy of the tricuspid valve and adjacent structures, selection of prosthesis size, and minimization of the complications risk. The presented experience with the Lux-Valve Plus prosthesis emphasizes the need for preoperative CT scanning and standardization of the examination protocol.
