Ogilvie's syndrome (OS), or acute colonic pseudo-obstruction, is a type of intestinal obstruction resulting in colonic dilation without mechanical obstruction. It occurs in both surgical and therapeutical patients. The most frequently reported cases of OS are those following abdominal and pelvic surgery. In this article, we will examine a case of pseudo-obstruction following left upper lobectomy. Of interest are the patient's MSCT scan results, performed due to a complication of the underlying condition, which revealed intestinal changes before the onset of significant obstruction symptoms.

Purpose. To demonstrate a clinical case of Ogilvy syndrome in a patient after an upper lobectomy of the left lung, detected at an early stage before the development of the clinical picture.

Materials and methods. Patient M., 71 years old, was diagnosed with peripheral cancer of the upper lobe of the left lung cT1N0M0 1 st. A planned upper lobectomy of the left lung was performed. In the postoperative period, due to suspected pulmonary embolism, multispiral computed tomography (MSCT) with intravenous contrast was performed.

Results. According to the radiation examination, signs of intestinal obstruction were revealed before the appearance of characteristic clinical signs with the presence of local edema of the colon wall in the area of the splenic bend. Later, with repeated MSCT, no mechanical obstruction was detected in this area, but signs of ileus persisted, which corresponds to the picture of Ogilvy syndrome.

Discussion. Ogilvie syndrome is a rare form of intestinal obstruction. Clinical manifestations are typical, as with any other type of ileus. Diagnosis is made using high-tech imaging techniques (MSCT) with characteristic radiographic features. However, in our case, the examination was performed before the development of clinical signs of obstruction, revealing changes that likely precede the classic presentation of Ogilvie syndrome.

Conclusion. Thus, MSCT performed for chest pathology revealed colonic changes likely characteristic of Ogilvie syndrome at a very early stage, before the onset of characteristic complaints that typically lead to abdominal examination.
