Purpose. To demonstrate a rare clinical case of emphysematous osteomyelitis in a comorbid patient by visualizing this disease using different modalities.
Materials and methods. A clinical observation of emphysematous osteomyelitis in a 66-year-old patient with type 2 diabetes mellitus and diabetic foot syndrome is presented. The diagnostic methods included clinical and laboratory data, X-ray, single-photon emission computed tomography combined with computed tomography (SPECT/CT), osteoscintigraphy.
Results. Due to the comprehensive approach to diagnostics, it was possible to accurately localize the pathological process and determine the volume of surgical intervention.
Discussion. Emphysematous osteomyelitis is a rare but potentially fatal disease characterized by the presence of gas in the bone tissue and in the surrounding soft tissues. A large number of immunosuppressive diseases predispose to the development of emphysematous osteomyelitis. At the moment, it is difficult to give an objective assessment of the true prevalence, since about 70 cases of this pathology have been registered in the literature.
Conclusion. For successful verification of emphysematous osteomyelitis, assessment of the true prevalence of the inflammatory process and determination of the optimal volume of surgical treatment, a multimodal approach is necessary, allowing to simultaneously assessment the anatomical changes and the metabolic activity of bone tissue.
Keywords: emphysematous osteomyelitis, diabetes mellitus, hyperglycemia, intraosseous gas, and infectious diseases.
