THE DIAGNOSTIC VALUE OF CT CHARACTERISTICS OF HEPATOCELLULAR CARCINOMA NODES DEPENDING ON ITS SIZE

Purpose. To determine the diagnostic significance of computed tomography characteristics of hepatocellular carcinoma (HCC) nodes of different sizes in the absence of cirrhosis.

Materials and methods.The study involved 55 patients (34 men and 21 women) aged 15-83 years which were undergoing surgery for HCC in the A.V. Vishnevsky Institute of surgery. Before surgery, multislice computed-tomography with bolus contrast enhancement was performed in all patients according to the protocol of the abdominal study with four phases of the study (native, arterial, venous, delayed) using Philips Brilliance 64 CT. The following parameters of tumor nodes were evaluated: localization, contours, presence of capsules, sighs of neoplasm contrast enhancement (homogenous or mosaic node parenchyma), and the presence of feeding and intranodules vessels.

Results. It is established that the intensity of the tumors contour depends on the phase of CT studies: at native phase node boundary was determined in 42.2% of the observations, at the arterial phase − in 54.7%. Signs of mosaic internal structure in the arterial phase was observed in 52.9%, 95.8% and 82.6% − in small, medium and large nodes, respectively. Areas of necrosis in the structure of tumors was determined at more than half (57,8%) of cases. Feeding vessels were visualized in 81.3% of all investigated HCC nodes. The increase of tumors in size was accompanied by an increase in both the number of feeding arteries, and blood vessels in the node structure.

Conclusions. Carrying out CT with bolus contrast enhancement allows to obtain a number of important CT characteristics of HCC nodes, the diagnostic significance of which depends on the node’s size.
